BIRTH CERTIFICATE WORKSHEET

Following the birth, the hospital birth clerk will use the parentage judgment which we obtained on your behalf to complete the birth certificate worksheet.  It will direct the registrar to allow you to name the child, as well as list you as the parent(s). 

If you are in need of having the birth certificate expedited, you should request the hospital birth clerk expedite the processing. 


OFFICE OF VITAL RECORDS

Once the birth certificate worksheet is completed, the hospital birth clerk will send the worksheet to the vital records office in the county in which your child was born. 

If the birth certificate can be expedited, you will ask the hospital birth clerk for the address of the local vital records office, as this will be where you will personally appear to retrieve the birth certificate.

Before making the trip to the vital records office, it is recommended you call beforehand to confirm the birth certificate is ready and available for pick up.


PURCHASE SEVERAL BIRTH CERTIFICATE COPIES

We strongly encourage parents to purchase several certified copies of the birth certificate.  This is especially important if you live abroad and/or will be having the child's birth certificate amended in the future.
